Experience

Essential:

• Experience of working in general practice or hospital frailty role with experience of care planning and admission avoidance work.
• 2 Years post qualification experience.

Desirable:

• Experience of working in primary care.
• A developed interest in frailty and long term condition management.

Qualifications

Essential:

• Full and current unrestricted GMC (UK) registration on the GP Register
• A medical degree qualification

Desirable:

• A teaching qualification e.g. post graduate diploma in post graduate education
• A post graduate qualification in older peoples care, e.g. Diploma Geriatric Medicine
